Health insurance costs include monthly payments, individual deductibles, drug coverage deductibles, out-of-pocket expenses, copayments, and coinsurance. For the Blue Transitions short-term health insurance plan, here’s what you can expect:

  • Individual deductibles for individuals ranging from $500 to $5,000 for in-network benefits and $1,000 to $10,000 for out-of-network services
  • Family deductibles ranging from $1,500 to $15,000 for in-network benefits and $3,000 to $30,000 for out-of-network services
  • You pay 20 percent of covered services from in-network providers and 40 percent for out-of-network providers
  • Emergency room visits have a separate $100 deductible per visit, in addition to the benefit period deductible, which is waived if admitted to the hospital
  • Monthly premiums based on covered person(s) home ZIP code, deductible, gender and age
  • To view your specific costs, get a quick quote.

Plan options provide you with the freedom to choose between various benefit periods up to six months, depending on how long you'll need coverage. If your wait becomes longer than expected, you may be able to re-apply for a second benefit period. You can also take advantage of one of our longer-term individual and family plans and transfer your unused premiums to that plan. Other plan highlights and special features include (but are not limited to):

  • The option to customize your health insurance coverage to your time frame and budget — choose a start date, deductible amount, payment options and benefit terms of one to six months
  • Blue Transitions pays 80 percent of the allowable amount for in-network covered expenses — you pay 20 percent after deductible is met
  • Emergency room visits in-network covered 80 percent — you pay 20 percent after deductible is met
  • Benefits that are greater when you use the services of doctors and hospital in the network
  • Please note: immunizations are not covered under the Blue Transitions plan

To learn more about what's included with this plan, view the Blue Transitions Enrollment Guide .

  • This is a single term, non-renewable, short term health insurance plan, and is not intended to be permanent.
  • If you need temporary coverage for an additional period of time, you may apply for a second Blue Transitions contract term.
  • If accepted, any pre-existing condition during the time of the first Blue Transitions contract will not be covered with the new contract.

Every insurance plan has limitations. These limits are there to keep health care costs down for everyone. A pre-existing condition is just one example of a limitation. A pre-existing condition may limit or exclude your participation in a health insurance plan. This means your health care expenses related only to that specific condition will not be covered. Please review your policy documentation for limits and exclusions.

  • Prescription drugs and medicines (not used in a hospital) are covered up to a maximum benefit of $750 per participant per benefit period.
  • There is a separate $200 prescription program deductible that must be satisfied before benefits are available.
  • Once the deductible has been met, you will be responsible for 50 percent of allowable charge, the amount based on whether your prescription is filled at a participating pharmacy or through the Prescription Drug Mail Service.
  • You can optionally order your prescription drugs through the mail with our 90-Day Supply Program.
