Location Requests Now Only Accepted Through the Demographic Change Form

As of May 20, 2024, Blue Cross and Blue Shield of Oklahoma will no longer accept additional location requests of existing providers through the Provider Onboarding Request process. Instead, please submit those requests using the Demographic Change Form.

Here are the steps to complete this request:

  1. Visit our Change Existing Demographic Information page. Complete the required fields.
  2. Select ‘Next’, located in the lower right corner of the screen.
  3. Under ‘Type of Change’, select ‘Add New Location’ and then select ‘Next.’
  4. Complete the required fields on this page.
  5. If you add comments in the ‘Additional Information’ section at the bottom of the page, the ‘Comments’ box has a 255-character limit, including spaces. Any characters over this limit will not generate the request. You have the option to include the additional information on an external document and attach it under the ‘Attach Documentation’ section. When attaching a document, please include the specifics of your request including group information, new service address information and roster of current providers, including NPI that are being added to the new location.
  6. Select ‘Submit Form’ once the requested information form is complete.

You should still use the Provider Onboarding Form when you are needing to add a new provider that is currently not linked to your group. Requests to add a location using the Provider Onboarding Form will not be processed.

Providers should continue to use the Demographic Change Form to update and terminate existing locations.
